The FCV Sentinel app stores data in accordance with the Terms of Use presented to you when you Sign In. The information we collect is encrypted and stored in a secure cloud environment.

Your data may only be shared only with the organization that provided you with access to FCV Sentinel. It is never traded with anyone, and it is never sold to anyone. During Sign In, you verify the following information provided by your organization's administrator or HR Department:

1. First Name
2. Last Name
3. Phone Number (Optional)
4. Date of Birth
5. Gender
6. Smoker Status
